Care Transitions Intervention: A Proven Patient-First Approach to Decrease 30-Day Readmissions

This session will concentrate on how a patient-centered approach within four areas of health management is proven to reduce the rates of subsequent rehospitalization as well as create lasting behavioral change and increased activation in patients and their caregivers during post-acute transitions. Topics to be covered include defining the care transitions intervention; emphasizing the value-add and return on investment a transitions coach has to the overall system, the care team and the community; and reviewing the unique strategies used within this empowerment model. Case-based scenarios will be utilized for attendees to practice the art of coaching. 

Presenters

 1Michelle Comeau is the National Program Director for Care Transitions Intervention. In this role, she supports the vast network of CTI Program Providers and Transitions Coaches stay active in CTI principals. She guides new partners through a readiness process to ensure successful implementation. Ms. Comeau has been working in evidence-based health prevention programs for over 10 years in various capacities; from local community initiatives, statewide program evaluation and development, and national coordination. She is proud of her purpose to help people help others in the most effective and fun way possible.
